Clinical and radiographic diagnosis. A new patient wanted dental treatment for a maxillary premolar tooth #13 that had been left exposed for over a year since root canal treatment was completed at another dental office. The temporary dental filling had fallen out long ago. 1. A dental x-ray radiograph showed root canal treatment that appeared to properly extend to the tooth root apices and a significant angular bony defect on the distal of the premolar leading to the first molar. 2. Buccal photo. 3. Palatal photo. 4. Occlusal photo. Treatment options: Tooth extraction and then dental implant tooth replacement or retreat the root canal because it was left exposed to oral bacteria too long and probably reinfected, then place a cast gold post and core, crown lengthening gum surgery and a dental crown. Based on the amount of dental caries tooth decay that extends below the gum as seen in the photos and the xray I would extract this tooth and place a dental implant.

Radiographic decay dental diagnosis. Teeth cavities can be difficult to identify on dental x-rays radiographs. Dark shadows can be seen in the outer whiter tooth enamel. Many dental students are taught x-ray diagnosis using film but more private practices are moving towards digital xray. Schools should better prepare students during these years of transition from film to digital dental xrays. I have personally seen this problem with dental students.
