How to pictures – Eggshell technique for making a full arch temporary – provisional – dental bridge. Full arch maxillary dental implants are seen in the photo on the left. An alginate impression was taken of the patient’s prior bridge. Acrylic liquid & powder were painted – eggshell thin – inside the alginate impression. After it hardened it can be relined first with hard acrylic to get a closer fit and then soft denture reline material. The lab can work out final parallelism for a new hard acrylic temporary after initial healing.

Fabrication of a lab-processed 11 teeth temporary provisional dental bridge as part of a Smile Makeover on a phobic Wall Street executive. Total treatment time was about nine hours in two visits during one business week: Monday (four hours) and Friday (five hours). This patient hid his dental condition for over a decade by never smiling.

In the first row of photos, note that the teeth bite was over-closed due to a prior history of an eating disorder, now controlled, and a current tooth clenching grinding habit.

An Oral Surgery consultation with panoramic x-ray (second row) was performed prior to treatment to evaluate opening the bite.

The third row shows x-rays taken after the Endodontist completed eight root canals on the first day on tooth #’s: 6 – 11 and 14, 15.

The fourth row shows the lab-processed 11 unit (11 teeth) temporary dental bridge and the cast gold post and cores (with Kaitlyn loops) for the root canal treated teeth.

The fifth row shows the cast gold post and cores cemented.

The sixth row shows the lab-processed provisional teeth bridge in place after Oral Surgery extraction of tooth #’s: 3 – 5 and 12, crown lengthening gum surgery was performed on tooth #’s: 6 – 11, and a distal wedge was performed on #15.

The patient will have a final porcelain-metal bridge made after the gums heal. Dental implants may also be placed in the upper right posterior. A bite plate is also necessary to try to mitigate the force of tooth grinding. Referral for pharmacological management of anxiety is also worthwhile.

A simple temporary for teeth veneers can be made using a clear acrylic stent that is relined with Luxatemp, which doesn’t cure exothermically. The second photo shows half of the provisional removed.

Second stage dental implant surgery following osseointegration. Uncovering the first stage dental implant abutments. Weak teeth #7 and 10 were left in the otherwise edentulous maxilla to help retain a fixed provisional temporary teeth bridge while four upper dental implants osseointegrated. These two teeth will now be extracted and a new provisonal temporary teeth bridge will be made while these remaining dental implants osseointegrate over the next four months. This sequence allows the patient to always have a fixed temporary dental bridge rather than a removable denture while the osseointegration occurs.

When performing oral rehabilitation dental reconstruction on a dental phobia anxiety patient it is important to give them a dramatic initial change as quickly and painlessly as possible. Crown lengthening periodontal gum surgery is performed to gain more tooth structure from below the gums. A barrier must be placed after this surgery is completed to prevent the gum from regrowing where it is not desired. This barrier is most commonly periodontal dressing placed by the Periodontist. Thereafter the temporary provisional teeth crown becomes the barrier, which needs to be lengthened by the dentist when the periodontal dressing is removed. Text below image reads “Temporary Crown.”

There is now a lot of tooth crown margin exposed above the gums because crown margins should not end on a core. After the temporary provisional crown is placed wait at least 6 weeks before taking a final impression for porcelain dental crowns.

Smile makeover involving full mouth temporary provisionals in a 40 year old male, married jeweler.
This patient had a very severe fear of dentistry so our goal was to recreate a handsome smile with temporary crowns as quickly and painlessly as possible. These before and after photos were completed in four days. More dentistry is still needed. This patient then referred his wife to us for a similar oral rehabilitation.
